My child is up-to-date on their immunization requirements for their age.

My child may have his/her picture appear in Day School advertising/promotion needs and UNTAGGED on the school Facebook page.
I understand this fee is non-refundable in the event we decide to withdraw from the wait list. I also understand that this amount will be deducted from the registration fee once I am offered a spot in the day school.
